How to appeal a citation

• To appeal the administrative citation, you must submit a request for an Administrative Hearing within fifteen (15) days from the date of issuance of the citation
• Complete this form(PDF, 25KB) and send it in with a deposit which equals the full amount of the fine.
• Mail or deliver the request and deposit payment to this address:
